Please take a moment to print and complete your patient forms prior to your dental appointment. You can email, mail, or bring your forms with you to your initial appointment. If you would prefer to complete your forms in our office, please arrive 15 minutes prior to your scheduled appointment time.

Dental Insurance, Payments & Financial Arrangements
We are in network for Delta Dental, Delta Dental Premier, and we also work with most other dental insurance providers. However, we are not taking any new patients with Medical Assistance, Minnesota Care, or other state funded insurance plans at this time.
We accept Visa, MasterCard, and Discover, we offer a 5% discount for payments made by cash or check on the date of service, and we offer patient financing through CareCredit.
